Medication orders for the 21-22 school year need to accompany medication and be signed by the healthcare provider and parent. Please label the medication with your child's name and Double check the medication to make sure it is the correct medication and it has not expired. If you cannot make these dates, please arrange a separate appointment with the nurse.
