Transportation Requests


1. General Information

This paperwork is necessary with or without a request for transportation.
  • Parent information - address(es), cell phone numbers, and email addresses.
  • Emergency contract information - names, addresses, and phone numbers.

2. Parent/Guardian Proof of Residency

Your current utility bill, lease, or mortgage statement is preferred. Please do not submit partial documents or envelopes. Other documents showing your name, address, and date can be considered in special circumstances, such as shared housing


3. Transportation Requests

Information and a form to request district transportation for private, parochial, and charter school students can be found on the Forms page of the Transportation website.
