Work Permits For Webster Thomas Students

For Thomas students 

  1. Print out the Application For Employment Certificate (AT-17) Forms can also be picked up in health office.

  2. Fill in the first part on page 1, including parent/guardian signature (use blue/black pen only, no pencil)

  3. Email them to both: and 

  4. Students will need a current physical examination on file (must be within 12 months) in order for the Certificate of Physical Fitness section to be signed by the health office. If your student does not have a current physical on file, you may email a copy of a current physical exam along with your application or you can have your doctor sign the Physical Fitness Certificate form (AT-16) verifying good health (must be dated within 12 months) and forward that along with your application. If necessary, you may fax to (585) 670-8023. 
    **Student must pick up and sign card in person at the Health office.


For Webster homeschooled and private school students in Thomas HS catchment 


  1. Print out the Application For Employment Certificate (AT-17) and if needed, the Physical Fitness Certification (AT-16)

  3. Provide a copy of a current physical exam (must be within 12 months) or a doctor signed Physical Fitness Certificate (AT-16).  

  4. Provide documentation of evidence of age (see form for info) 

  5. Provide copy of a photo ID

  6. Email all items to both and

  7. The student must be available to pick up the work permit in person.( BY APPOINTMENT ONLY)  Appointments are available Monday - Friday during the hours of 7:45am and 3:15pm.  Call 670- 8011.
