Work Permits For Webster Thomas Students

Thomas Students 

1. Print out the Application For Employment Certificate (AT-17) available under Downloads.
2. Fill in the first part on page 1, including parent/guardian signature (use blue/black pen only, no pencil).
3. Email Ozlem Kus at ozlem_kus@webstercsd.org or Allison Coulomb at allison_coulomb@webstercsd.org.
4. Students will need a current physical examination on file (must be within 12 months) in order for the Certificate of Physical Fitness section to be signed by the health office. If your student does not have a current physical on file, you may email a copy of a current physical exam along with your application or you can have your doctor sign the Physical Fitness Certificate form (AT-16) verifying good health (must be dated within 12 months) and forward that along with your application. If necessary, you may fax to (585) 670-8023. 

Home School & Private School Students in the Webster Thomas HS Catchment 

1. Print out the Application For Employment Certificate (AT-17) available under Downloads and, if needed, the Physical Fitness Certification (AT-16).
2. Fill in part 1 on the Application for Employment Certificate, including parent/guardian signature (use blue/black pen only, no pencil).
3. Provide a copy of a current physical exam (must be within 12 months) or a doctor signed Physical Fitness Certificate (AT-16).  
4. Provide documentation of evidence of age (see form for information).
5. Provide copy of a photo ID.
6. Email Ozlem Kus at ozlem_kus@webstercsd.org or Allison Coulomb at allison_coulomb@webstercsd.org.
7. The student must be available to pick up the work permit in person. Appointments are available Monday – Friday, 7:45 a.m. – 3:00 p.m.
