Work Permits

Application for employment for farm work or news carrier – please stop by the health office and pick up a special permit form.

For child modeling – must apply online at http://www.labor.ny.gov/workerprotection/laborstandards/secure/child_index.shtm

General Employment – print out the two (2) forms, fill in the top portion of both (use pen only, no pencil) and return them to the Webster Thomas Health Office during the school year. (In the summer, turn them in to the main office.) See below for DOWNLOADS -TO PRINT OUT BOTH FORMS.

Student will need a current physical on file (physical date must be within one calendar year to be valid) in order for the Physical Fitness Certification form to be signed by the Health Office. Or, you can have your doctor sign the (Physical Fitness Cert.) paper verifying good health.

For Thomas students – fill out top half (part 1 section only) of the Application for Employment form, and the Physical Fitness Certification (top-student name, address, DOB). Return both forms to the Health Office.
 
For Webster students attending other private and parochial schools – you must fill out Part 1 and Part 2 of the Application form and provide documentation of evidence of age (see form for info) and fill out the Physical Fitness Certification. Return both forms to the Health Office.

STUDENT MUST BE PRESENT TO PICK UP AND SIGN THE WORK PERMIT CARD.


If you need a duplicate card for a second job and the student has not changed age. Please call the health office for more information, or if you have any any questions phone 670-8011.
